I authorize King Jesus International Ministry to contact all individuals, organizations and references
listed on this Application in order to verify the information I have provided. I agree to release from
liability any person or organization providing information related to me, including those persons I
have listed as references, as well as contact persons from my previous volunteer work or
employment with children. I specifically authorize King Jesus International Ministry screening
personnel to speak with any past employer or organization where I have volunteered or been
employed related to working with children, minors or vulnerable adults. I authorize King Jesus
International Ministry to undertake a criminal background check concerning my past.

I hereby grant to King Jesus International Ministry its legal representatives and assigns, the right and
permission to use my name and likeness and to use and publish photographic pictures, video
footage and sound recordings of me during church related events, whether via print, digital,
broadcast or in any other media and for any lawful purpose whatsoever, at their discretion. I
RELEASE, DISCHARGE, AND AGREE TO HOLD HARMLESS KING JESUS INTERNATIONAL MINISTRY
FROM ANY LIABILITY FOR ANY USE OF THESE PHOTOGRAPHIC MATERIALS REFERENCED ABOVE.

Further, if I serve as a volunteer photographer, videographer, writer, songwriter, singer, artist, musician, performer, graphic designer, creator, curriculum writer, stylist, or any other position where I create or perform for King Jesus International Ministry for any of its services, activities or events (the “Activity”), in exchange for my participation, I
agree to assign all ownership in the photos/videos/content/creative materials created by me during
the Activity (the “Materials”) to King Jesus International Ministry for any lawful purpose through any
media whatsoever; I acknowledge that the Materials will be considered a “Work Made for Hire” as
defined under U.S. Copyright Law. I further agree that I will not display, use, distribute or sell the
Materials without permission from King Jesus International Ministry.

I understand that COVID-19 is an ongoing pandemic in the United States and around the world and that the
transmission of COVID-19 can be passed from person-to-person in close contact. I acknowledge that
participating in the Activity will put me in close person-to-person contact with King Jesus International
Ministry staff, attendees and other volunteers and I assume complete and full responsibility for any COVID19 transmission that may occur.
